Empowerment

Giving people the knowledge and skills they need to make healthy choices. Empowerment should make people feel in control, less afraid of their decisions

Empowerment means helping people believe they can take care of themselves and make healthy decisions.

 

Chronic Disease Community

People living with long term health conditions (diabetes, heart disease, asthma) AND the families, caregivers who support them.

 

Health Literacy

The ability to understand health information (education). This would include reading medication labels, making healthy food and activity choices, understanding instructions from healthcare providers, being able to ask questions at doctors’ appointments.

Health literacy basically means being able to understand health information/education and use it to care for oneself.
